Same thing is happening to me as well. Two in a row before I stopped playing. Same issue with the tokens going to graveyard sometime as the card that spawns them.

It seems people can force a draw by choosing a cost for momir and cacncelling. Some people do this rather than lose.

100% repro: activate the Momir ability, then hit the Cancel button.

Same here, I found out how to (unreliably) reproduce the bug, but I do not know what is causing it.
To reproduce, you have to activate the Momir Emblem, choose X, then select a land to discard, but now click "Cancel" instead of "Submit". Do this few times over, if it doesnt happen, go to next turn and try again. For me it usually happened in Turn 2 or 3 with 4-5 "Cancels" per Turn. Pretty gamebreaking if it is reproducable IMO.
