Able to see the top card of my library when I shouldn't
I had Radha, Heart of Keld on my battlefield, and I was able to see the top card of my library. After Radha died, the top card of my library was still revealed, which it presumably would be until I drew that card.
However, after my opponent forced me to mill several cards, the top card of my library was still revealed until the end of that turn - that is, I could see the other cards that were now at the top of my library despite Radha, Heart of Keld being dead. This lasted until the next time I drew a card, through multiple instances of milling. I won the match, by the way.

I had a similar problem with Halo-Charged Skaab. I played a Halo-Charged Skaab and put an instant on top of my library. Then on thier turn my opponent played a Halo-Charged Skaab and milled that card. Then for the rest of the game I could see the top card of my library.
