Hundreds of Scute Swarm entering play did not trigger Divine Visitation. It created scute swarms instead of angels
It also did not trigger Bishop of Wings with Maskwood Nexus in play, so I did not gain any life (i should have got Hundreds of life)
But creating just 1 token in other ways triggered correctly. It's just the massive Scute Swarm token creation that did not trigger anything after 248 tokens
