Wasn't given the opportunity to respond to trigger
This is a borderline bug, more misleading UI than anything. I cast Blink of an Eye, opponent responds with Kinnan's ability. I click "Resolve all". Kinnan's ability brings out Thorn Mammoth, which creates a fight trigger. The trigger resolved without letting me respond, despite that fact that I had multiple options in hand to respond, sufficient mana, and no restrictions in play that would prevent such a response. This happened because I clicked "Resolve all" when opponent activated Kinnan's ability. However, if Thorn Mammoth entering the battlefield created a new trigger on top of the stack, the game should give the option to respond to that new trigger, generating a new instance of the "Resolve or Resolve All" dialog, rather than just letting all new triggers sneak through when they weren't on the stack at the time user selected "Resolve all". To be clear, I could have just clicked "Resolve" when Kinnan's ability went on the stack, but it stands to reason "Resolve all" would mean "Resolve all triggers that are currently visible on the stack" rather than "Resolve all triggers currently visible on the stack, as well as all triggers that may be generated as a part of this process, regardless of what those new triggers may be."
