Mondrak, Glory Dominus and Vesuvan Duplimancy breaks the game
The problem is an interaction between "Mondrak, Glory Dominus" and Vesuvan Duplimancy. Due to the exponential nature of Mondrak, if you cast a spell that targets Mondrak while Vesuvan Duplimancy is on the field, it leads to weird behaviour of the game. I think I read somewhere that there is a cap on tokens created, although even with ~250 Mondrak tokens, the game often just abandons, leading to a draw, or goes back to the main Menu. When copying a different creature while ~250 Mondraks are on the board, the game just abandons the process without any hint. which is understandable - it would have to create more tokens than atoms in the observable universe - but I think it would be good, if there was at least some kind of hint, that something weird happened.

Oh I had someone create 250 tokens all indestructible and had mondrake at 24atk and 24 def these cards should be banned from standard until they fix this bug it's over powered

This combo is Exponentially broken. I got hit with this twice in one game x*2 is NOT the same as 2*x :)
The first time the combo triggered, it created 10 new Mondrak. Then the next turn it triggered again and created 239 additional Mondrak token cards. -

Casted and flashbacked "Homestead Courage" in the same turn with Mondark and Vesuvan on my field and created 250 copies of Mondark. Still would've won without the bug but I abandoned the match due to being unfair. My rank-up win ='')

There is a hard limit of 250 tokens per name, so that makes sense. But it should still be possible to execute this combo without freezing the server.
I also observed some strange behavior with this bug, where attacking an opponent with 15 life with my single original Mondrak caused the life total to drop to 7 before the card hit, and then -1 after it combat. This part may have been a front-end glitch, but it's hard to say for sure.
