Board state: Ojer Kaslem and Mythweaver Poq are on the battlefield.

Action: attack with Ojer Kaslem, causing one or more damage to opponent, and then select a Forest in your library to add to battlefield under your control.

Expected behaviour: Mythweaver Poq’s ability triggers, which conjures an additional Forest to the battlefield under your control.

Actual behaviour: Mythweaver Poq’s ability doesn’t trigger at all.

I’ve attached screenshots showing the battlefield state after all the above actions. Note that only a single untapped Forest has been added to the battlefield. Also note that Ojer Kaslem died in combat, so it’s been transformed as Temple of Cultivation (its abilities still triggered, and all the above steps were still followed) — I suspect it’s this nuance which is where the bug stems from.
