Moss-pit Skeleton trigger does not check that card is back in the graveyard
When I trigger Insidious Roots with a Moss-pit Skeleton in the graveyard, a Snarling Gorehound on the battlefield, an Insidious Roots on the battlefield, and at least one pre-existing plant token on the battlefield, Insidious Roots resolves creating a new plant token and puts a +1/+1 on all of my plant tokens. I correctly get 1 Snarling Gorehound Trigger and several (603.2c) Moss-pit Skeleton triggers. I resolve 1 of the Moss-pit Skeleton triggers leaving the other Moss-pitt Skeleton triggers at the bottom of the stack. Insidious Roots triggers again making more plants and triggering Snarling Gorehound again. After I resolve these new triggers surveiling Moss-pit skeleton back into my graveyard, the first Moss-pit Skeleton triggers are removed from the stack without giving me an opportunity to put Moss-pit Skeleton back on top of my library. I should still be able to resolve these triggers, because the game should check the intervening "if" when the ablity triggers and when the ability resolves (603.4.) In this case, the condition is true when it triggers and at resolution.

The Moss-Pit Skeleton is NOT in your graveyard anymore. Instead an identical looking object is there, but that object is a different one as far as the game is concerned. Because whenever an object changes zones, it becomes a new object. So the intervening-if condition is not true anymore, and thus the triggers don't resolve.
