Calamity, Galloping Inferno creates two tokens instead of one.
Calamity, Galloping Inferno (OTJ 0116) created two tapped and attacking creature tokens when I think it should only be making it once. on my main phase, I saddled it with one other creature. When I declared attackers, two tokens were created. This happened 4 times over three matches.

When Calamity, Galloping Inferno attacks while mounted, the ability should trigger twice but only once and only one token is created. This bug may or may not occur.

I have noticed this bug as well (see pic1). However if you use Doubling Season it works correctly (see pic 2). I also repeated this many times with Sparky, with Invasion of Zendikar on the battlefield.

Calamity only triggered once. I used Terror of Towashi to saddle, who was enchanted to be a 0/1 no abilities. Terror created one token of Terror as 4/3 unenchanted. Both were able to attack Planeswalker the Eternal Wanderer. Where only one should be able. That happened three times. Then I tried to attack the player saddled with generous Plunderer. It still only made one token.
This is on iOS.

I also encountered this today with a planeswalker on the battlefield. Had Calamity, Galloping Inferno saddled and attacking with an Annie Joins Up on the battlefield and I think the initial trigger triggered twice (due to Annie Joins Up), but the "Repeat this once" triggers didn't - I got 2 copies instead of 4.

I have had this bug happen as well when an ETB triggers when the first creature triggers.

Calamity, Galloping Inferno does not create the second copy of the creature that saddled it if I am to declare what object to attack with the first copy. (when there is Invasion of Zendikar on the field in this instance, but I assume it will behave the same if there is a planeswalker that I could be attacking)

Calamity, Galloping Inferno, when saddling a creature and opponent has a planeswalker out, only 1 token is being made instead of 2. I've heard this also happens if opponent is defending a battle. I was on Android mobile when this happened. I believe this happens since it wants you to chose where the tokens are attacking but then doesn't produce the second token

Same thing happened to me. When attacking Oko with Calamity, I only made 1 token. In another game without a planeswalker, I successfully made 2 tokens.

I can confirm that whenever Calamity triggers when there are 2 or more potential attack targets, making you pick which ones to attack, only 1 token gets produced.

Just lost a draft because of this and I guess I'm just SOL

Calamity does make two tokens, but is currently bugged when there is more than one attack target (player, planeswalker, battle). Something in the game logic causes it to skip the second token when assigning the attack target for the token.

That is how it is intended to work.

As the title says, Calamity, Galloping Inferno on creates one token copy when attacking a player defending a battle (or attacking a player that controls a Planeswalker, according to mja16777216#38339).
This bug is repeatable easily against Sparky. The bug seems to occur as part of selecting targets for the new tokens. If no additional target is available, it functions as intended.
This was on Arena on Windows.

When attacking with Calamity, if only saddled with one creature he will only make one token copy. He should allow you to make two copies of the same saddled creature.

"Repeat this process once" means that yes, two tokens should be created. It's a busted card!
(Not a WOTC employee, I just came across your post when I came to report my own bug at https://feedback.wizards.com/forums/918667-mtg-arena-bugs-product-suggestions/suggestions/48271433-calamity-galloping-inferno-saddle-triggers)
