Chandra , Awakened Inferno -X ability didn't exile creature.
Used Chandra's -X ability to kill Atsushi, the Blazing Sky. The creature died, went to graveyard and triggered its death ability instead of putting it in exile. I believe this is an error because Chandra, Awakened Inferno Reads " Deals X damage to target creature or planeswalker. If a permanent dealt damage this way would die, exile it instead."

Did you a Soul-Scar Mage in play? It replaces damage with -1/-1 counters, and the exile effect won't happen.
