Deathtouch and Phlage interaction?
With a Rev, Tithe Extractor and Isshin, Two Heavens as One in play and an escaped Phlage, Titan of Fire's Fury, I attacked with Phlage. I believe I stacked the triggers correctly to give Phlage deathtouch before its attack triggers resolved, making it so that its damage should have had deathtouch, but my opponent's Ghalta did not die.

You can tell if you stacked the triggers correctly, when the granting deathtouch trigger resolves first. If it is still on the stack when Phlage deals its noncombat damage, you ordered them wrong.
In game, you see the bottom triggers on the left, overlayed by the triggers close to the top. So the rightmost trigger is the one on top.
