I had [[Joshua, Phoenix's Dominant]] in play and activated his transform ability. In response, my opponent cast [[Eject]], which should have returned him to my hand. Instead, Joshua disappeared. He was not in the graveyard, in exile, or my hand. This was on Android, so I don't have the log files unfortunately. But this absolutely cost me the game.
