Cast Into the Flood Maw with gift targeting opponent's Elspeth.
Instead of one token it made two.
That caused me to lose the game a few turns later.

Because of Elpeth's ability. The token is created before the planeswalker is bounced, so the replacement effect applies, making two tokens instead of one.
