scury oak
scury oak gains a counter without other cards present to give him the +1/+1 counter. I don't know which combination is triggering this but I had my scury oak, bishop of wings and thunderbond vanguard. as soon a a token is created scury oak gets a counter and creates another token. I would expect this when using cathat's crusade, it was the whole combo idea but that happens even without it being in play.

The key is the Scurry Oak's evolve ability. When Thunderbond Vanguard enters while the Oak is already on the field, the Oak will evolve. This gets it a +1/+1 counter, and triggers the token creation. And each token created is a copy of Thunderbond Vanguard as it enters, so the new token always has higher power than the Oak, and thus the Oak gets to evolve, meaning it gets a counter. And the loop is complete.
