tokenized Sword of Vengeance fails to equip
Twice I have seen a token copy of Sword of Vengeance fail to attach to a token creature after selecting to equip it. I don't think there's a reason it should fail.

Depends on how the token is created. If the token copy is also a creature for example, then it cannot get attached to anything. Because creatures are not allowed to be attached to anything.
