I had Blade of selves equipped to Mycoloth, and on the battle field were roughly 70 saproling tokens that had all been generated by the equipped Mycoloth. Once combat was over the Myriad exile effect took place. Rather than exiling just the Mycoloth token, it also took with it all 60 attacking tokens and the 10 defenders I left up resulting in a loss that should have easily been a win on my next turn.

Jegantha in the sideboard of a deck (modern tron) that meets the companion requirement (No card in the starting deck has more than one of the same mana symbol). Magic Online says the deck doesn't meet the requirement until I remove the 1 main deck Walking Ballista, at which point, Jegantha is highlighted blue to indicate that it is a valid companion.1 vote

Commander game, I have my commander (Polukranos, Unchained) in my graveyard. I checked the zone-change settings, I have "Command zone, instead of library" selected.
My opponent casts the Aftermath-half of Commit/Memory which says:
"Each player shuffles their hand and graveyard into their library, then draws seven cards."
What should happen:
Polukranos goes to command zone. I draw 7 cards.
Polukranos went to my library, I drew 7 cards, one of which was Polukranos .4 votes
Check out the attached, this happens to me too often. I have other screen shots as well. Ran it through a hypergeometric calculator. Feel free to do the same. 0.000028712 are the odds. About the same as winning a high stakes lotto. Please fix.1 vote

Had a hexproof 3/6 (Wingfold Pteron) that had 3 mutates on it, at the end of my op's Turn I flashed mutate a pouncing shoreshark onto the bird, drew a card for Dreamtail, targeted Luris with bounce and fetched a swamp for Greathorn, and this interaction killed my creature2 votes
sakashima in play copying a creature and being target by oko from the oponnent
When this happened MTGO made me choose a Sakashima to keep and even though I tried to click the creatures several times it didnt let me continue or cancel.2 votes

I know you can select anytime before hand where your commander will end up "before hand," but this is flawed. Where I may want mu commander to go depends greatly on what my opponent does if they kill, exile, bounce it, etc. this setup can also cause many dumb mistakes, like killing your commander with your own Thasa.
Can it be switched so that when you commander leaves the battlefield, you are then prompted to select where it goes? Like in real life? This would eliminate mistakes and allow reaction to your opponent play3 votes
I just recently had an instance where upon casting Unmoored Ego the end search does not go away and the view battlefield icon is still accessible throughout the match.2 votes
Umbral mantle trigger ability can interrupted equipied mana creature.
1)Equip Umbaral mantle on mana creature.
2)Tap to mana creature and lost mana.
3)Umbral mantle trigger ability use to mana creature(need untap & pay3 mana).
4)ONLY untap mana cureature and NO pay mana cost.
5)Interrupt Tap to mana creature, it add to mana cost.
6)Add mana can pay to Umbral mantle trrigger cost.

Avatar - Elvish Champion in Vanguard results in every game being a draw before opening hands are even seen. In Solitaire it causes a win, but regardless it makes it impossible to play with it as a Vanguard.3 votes
I have lost too many times now to time that I have to post a thread about this. Just last game, I was about to win that same turn and couldn't finish game actions before timer ran out. I don't stall or go afk. I do put thought into my gameplay decisions. I use the pass priority hotkeys. One should be able to complete a match playing attentively with very few instances of timing out. This is a major disadvantage to control and combo players. In casual this wouldn't be an issue, but in tournaments, where multiple prizes are on the line, it's very frustrating. I've given up entirely on drafting storm in Vintage Cube simply because of how many times I've timed out. Again, players should be able to think through their turn and execute their gameplay without worrying about losing to an arbitrary timer. Since the switch to a 25 min timer, I've probably timed out of at least 30 or 40 games. This really adds up when you factor in prize payout. I understand the change to a 25 minute timer in a traditional draft setting (before leagues returned), as we were often waiting quite a while for the next round to begin. But with league play, why do we still have the 25 minute timer? I think it's to line Hasbros' pockets. The shorter the games, the more queues we jump into. BTW, I've never timed out on Arena. Bring back 30 minute timers to MTGO!
