An error occurred while saving the comment Heart#97945 commentedTo narrow this down further, this happened specifically because your opponent controlled a Planeswalker. This also happens if they're defending a battle.
Anytime you have to make decisions for what the tokens are attacking, it breaks the second copy.
Huge bug, still no traction on it being fixed. At this rate, I don't think it will ever be fixed.

Have you tried again since fixing with the sideboard? Making sure there were no extra spaces or line breaks?

@DAC169#34537 I'd assume they mean opponent's Ashiok (as Ashiok only exiles opponent's graveyards), and the OP's Case (since that's the most likely scenario
@Sakk cases can only be solved on your turn. So if the opponent activates Ashiok (their turn 99.99% of the time, your case won't solve because it's not YOUR end step

If you conceded, then how would you know what one of the disguised creatures is? That information isn't revealed when you concede.
I will say, morphing/disguise cost to flip a creature face up is a special action and doesn't use the stack. Rattlechains has no triggered abilities on morphing (as usually it can't be cast face down). So they could flip it face up and cast Supreme Phantom basically all in one action.

I ask because looking at the log it looks like there was a High Noon in play which would have stopped you from casting if you had already cast a spell that turn

An error occurred while saving the comment Heart#97945 commentedThis bug has been going forever and it's such an easy, gamebreaking bug to replicate. No reason it shouldn't have been fixed already.
I've noticed this too with draft sometimes (usually a negate).
A work around I've found is to drag and drop the card into your deck and it will drop the appropriate one there and you can add to it as normal.
