    This bug is triggered when a battle is dealt damage to kill it and let you transform it. After casting, the UI allows an option of casting from the command zone, and from Exile, showing the commander and the battle. If the commander is not cast, after casting the battle, the commander disappears from the command zone. They will also not be present in Exile or the Graveyard.

    Casting a second battle will provide the same UI a second time. This looks like it's mostly a UI bug of some sort.

    This bug is reproducible with any flash commander, and any battle.

    It's really frustrating to see communities that are working to host events in the format as defined by Wizards, but can't actually do it. Friendly Brawl is a neat idea, but it would be better if it didn't replace cards that were already in your deck.

    Elder Dragon War has the mechanic "Read Ahead". This allows the saga to enter play with a number of saga counters of the players choosing on the card. It sounds like your opponent cast the card and chose to have it enter with 3 counters. This allows them to skip the first two parts of the saga and just create the 4/4 dragon.
