Item is an Ostoria lore tablet. It is found behind the chest in the ice cavern in the northeast corner of the screen in the sixth encounter in Act II of the Kessel series. The tablet shines as though it's meant to be collected, but pressing the X button while standing over it only triggers the chest-opening animation. The tablet itself cannot be collected.
My co-op buddy made the same musing as Fooga regarding a counter for the lore items. Sound idea; I'll second (or third) it.
