Same thing has happened to me several times at this point. This resulted in a loss of each game that would have probably been wins otherwise. Incredibly frustrating. I think it has happened mostly or only with adventure spells for me. The most recent time (in attached logs) is when I was trying to counter the creature half of an Adventure spell. It asks to select target spell to counter (which it usually selects itself automatically on the Stack) then when I manually select the spell (the only other spell on the Stack) it let's me pay for it and then sends the Phantom Interference to the Graveyard but leaves the spell uncountered. With a bug like this you either have to stop using this card altogether or resign yourself to a loss when it happens. Please do something about this.

This has happened to me a few more times today. It only seems to happen when there is something else on the stack besides the spell being countered and Phantom Interference. Just now it happened when there was a creature being cast, followed by a triggered ability to that creature entering, followed by the Phantom Interference counter spell mode. This time I tried letting the triggered ability resolve to see if that would circumvent this bug. No luck. This card is completely unplayable.
