    I have confirmed the bug happens with any ability thay duplicates tokens and has to do with Hofri giving EACH token the ability that makes the exiled creature return to the graveyard when the token leaves the field. The system is unable to keep track of the exiled creature and when it return it as multiple tokens may be in different locations. This is an essential strategy for this commander and this bug makes it close to unplayable.
