This is correct. When you crewed the Forge, it became a creature with no power or toughness, so it went to the graveyard as a state based action

A common misplay with Exploit. If you Exploit a creature, but the original creature is not on the battlefield as you resolve (it has left play but you still sacrifice a creature), then you won't get the trigger from the sacrifice.

I am trying to figure out what format you have this deck set to. Standard Brawl doesn't include the LOTR set, and Friendly Brawl is a 40 card format.

You activated the land a step too late. The game was already in the declare blockers step, and you need creatures as that step begins to assign them as blockers.

Boros Recruit must attack, but you cannot pay the cost to allow that attack to happen. Typically effects that say you cannot do something win out against effects that say you must do something.
