His trigger is going off before the target actually dies.
Example:
I have a creature with the counter. My opponent casts a spell, targeting my creature, which will kill that creature. But, as soon as the spell is cast, the trigger for when the creatures *dies* is added to the stack. This is incorrect, even when I do not have a response. The permanent needs to hit the graveyard *before* his trigger can be added to the stack.
I understand if the game is taking shortcuts in the event that I did not have a response. But, with how magic is supposed to work, this interaction is not correct. Log included in case it helps.

Log attached in case it helps, most recent game was against this deck.
There were multiple instances where my creature was targeted with a removal spell, with a counter on it, and his trigger would happen as soon as the spell was cast.
It shouldn't happen until the permanent has gone to the graveyard, and it left the battlefield while having the counter (since the counter could be removed before it left the battlefield)
