If an object leaves the battlefield and then returns, it is considered a completely new object from the one that was previously on the battlefield. The once per turn limit will not be considered by the new object.

Permanents can be attached to permanents you don’t control and vice versa. Otherwise cards like Pacifism just wouldn’t work.
Helm of the Host says “At the beginning of combat on your turn”. It’s not its controllers turn.

Are you attempting to counter spells that can’t be countered because of Chimil or Lier?
Was Reflection of Kiki-Jiki copied by something like Glasspool Mimic? Cards that copy the backside of a transformed side won't copy the cards mana value. It would have been considered 0 in this situation.
