When I had something similar happen to me recently it was only a visual bug. The game-end screen displayed "Draw", but when I went back to the event page I was credited with a win.

Opponent had (at least one) Progenitus in the deck. That card reads "If Progenitus would be put into a graveyard from anywhere, reveal Progenitus and shuffle it into its owner's library instead."

This includes milling. So every time Progenitus got milled, it got unmilled, and after all your Jace triggers and copies the Progenitus/es was/were still in their deck.

Had this one happen to me today in a Golden Sealed match. Opponent hit with Incinerator and an empty graveyard and timed out, at which point the game went a bit haywire with rapidly flashing triggers.

Closing and reopening Arena took me to the "Entering match" screen (displaying my Username and that of my opponent), but the program hung there. Reopened it again ~7 minutes later, and was still in the haywire triggers match screen, which I was able to concede from.

One think to watch out for: There's a difference between exile spells and exile effects.

To compare two of the exile cards commonly used in standard at the moment: Lay Down Arms is a sorcery. The spell targets a creature as it is cast. The spell Ossification is an enchantment. The target of the spell is a land. The enchantment creates an exile effect targeting your creature.

Rotpriest triggers off of spells targeting your creatures. So if opponent Ossifies your Rotpriest, that won't trigger the Rotpriest -- it wasn't targeted by a spell; only an effect

This has occurred multiple times to me during premier drafts. I can't give a repeatable reason when or why it happens.

Normally during a draft, single clicking a card "Reserves" it. You can either double click or select "Confirm Pick" to select it, or it'll be selected automatically when the timer runs out.

However, there are times (roughly 1-2 per draft) where single-clicking the card simultaneously selects and confirms it, so the card ends up picked instead of just reserved.

First time I encountered this bug. Occurred for me in a Historic ranked Bo3 match. Only thing that appeared on screen for me was the settings gear in the upper right corner. I concede game 1, and the normal sideboarding screen appeared. After sideboarding was finished, the exact same "settings gear only" screen appeared.

Apparently the match itself has loaded. By clicking randomly on the screen I can manage to select whether to play or draw first. I just can't see any of my or the opponent's cards.

Attached log is from game 1, prior to my concession.
