Didn't receive mythic card reward from mastery

I was playing standard ranked and was going to hit level 29 on the mastery pass rewards. After the match, I gained enough experience for level 29 but I never received the mythic card reward from mastery. The Zendikar Mastery shows that the mythic reward has a check mark over the mythic card reward but I never received it.

Did Not get Minus Tirith and the Eowyn Avatar from Mastery Tree. Tree shows I did!

I just experienced this when reaching level 46 with the Mastery track. I have a feeling this has happened at least once before.

I haven't received Mythic Rares on the paid Mastery track many times now, I just watched it happen to be completely certain.
