Phyrexian Fleshgorger Ward Bug
I believe there is a resolution issue with Phyrexian Fleshgorger. I have no video of this match unfortunately, but the boardstate and actions were as followed:
I both owned and controlled a Mishra, Tamer of Mak Fawa, and had a Phyrexian Fleshgorger in my graveyard. The opponent had exactly 11 life.
The opponent resolved a Titania's Command, selecting to Create 2 Bears and give +1/+1 counters, resulting in two 4/4 Bears.
I use Mishra's ability to unearth my Fleshgorger for 1BR, move to combat, and attempt to swing for lethal. The opponent responds before blocks are declared by casting Abrade, targeting Fleshgorger with the "Destroy target Artifact" mode. The Wards of Fleshgorger and Mishra enter the stack, and the opponent pays the Ward cost for Fleshgorger, immediately falling to -3 life and losing the game. Initial interpretation could be the layering of multiple Ward abilities and duplicating the costs, as the resulting -3 life lines up with paying 7 life (Fleshgorger's power) twice for 14 life.

This exact thing happen with my duel against humans where i had to pay ward twice on a creature with 2 ward one one it making me pay 4 mana to cast the spell

Seems like the bug might be any instance of Phyrexian Fleshgorger's ward and one other ward ability on the stack at the same time.
Here's a video with timestamp where a streamer triggers the bug by targeting a Fleshgorger with two burn spells, then the first ward ability incorrectly gets doubled to 6, but the second ward correctly costs 3.

I have experienced this exact same bug several times. It seems to always happen when you have Mishra, Tamer of Mak Fawa and Phyrexian Fleshgorger out on the battlefield at the same time, I don't believe there are any other factors. Like you suggested, every time I've experienced it the ward cost is always exactly doubled.
