Simultaneous triggered effects
When two things happen simultaneously, they go on the stack separately, causing wrongful match losses.
Example, this has been a more common occurrence lately, but if both players have Sheoldred the Apocalypse on the field, and one player is at 2 life if that player draws a card, they will lose the game, because the game resolves the simultaneous life loss and life gain separately, which is incorrect. There have been other instances of simultaneous life gain and life loss happening incorrectly, but I forget the names of the cards. Certain black enchantments that cause life loss when creatures enter the battlefield with cards like Prosperous Innkeeper for example.
Wins or losses shouldn't be decided until the whole stack is resolved, for exactly this reason.

It is not incorrect. Multiple triggers resolve in APNAP order (Active player Non active player). The non active players triggers will go on the stack last and resolve first
