If this card is applied to a creature that dies in prototype form, the creature is returned in full form. I’ve seen this a few times. Is it intended?

Yes. Any prototype card put on the field other than from the stack by being cast for the prototype cost will put it there as its full fledged version. Be that reanimation, blinking, simply casting for the full cost, or whatnot. Even if the card has been cast previously for the prototype cost, the card itself retains no memory of that, it's not a perpetual effect.
