Phyrexian unlife bug
I was playing against an opponent on Magic the gathering arena who played phyrexian unlife. They did not receive any poison counters after taking damage after reaching 0 life.

Also, they have to take DAMAGE. Merely losing life doesn't count.

I can almost guarantee that they had a Solemnity out.
