Incorrect interaction between Quina, Qu Gormet and the Lasting Fayth adventure
With Quina, Qu Gormet in play I cast the adventure side of the card Zanarkand, Ancient Metropolis. I was expecting to create a 7/7 hero and a 1/1 frog. I instead created a 7/7 hero and a 7/7 frog. I can't find a way to interpret this result while reading the cards so I can only assume that it's a bug.

I had the same thing happen. I won the game, but my opponent made a 9/9 hero and a 9/9 frog with Quina and Zanarkand. I'm not sure how the bug is happening but my guess is it's something wrong with ability of Zanarkand.

This has also happened to me twice during drafts leading to a swift defeat. The comment below states that the token creation with additional tokens is copied. But, that doesn’t seem clear to me as the Qu card states that a token entering triggers the 1/1 and doesn’t say “if a token enters, copy the effect that created the token”. Any token entering should only create a 1/1. The additional counters are a second sentence, too, not “create a 1/1 token with x counters on it”. It seems to be an incorrect interaction.

No, not a bug. If a token creating effect also applies additional effects onto that token, then any additional tokens created by the same effect through a replacement effect will also be under that additional effect. Since Lasting Fayth creates a token and then puts counters on it, the additional token created via Quina will also get those counters.
