scythecat cub
Scythecat cub counter is not placing its +1/+1 counters on creatures intermittently. Typically occurs when Kodama of the West Tree's ability triggers -> basic land placed on battle field tapped -> scythecat club triggers ability. When choosing to place the counter on Kodama or other creature, the +1/+1 counter from Scythecat club does not actually place on the creature chosen (only when other than scythecat club).

I would like to add onto the comment also. Scythecat cub was not able to add a +1/+1 counter to multiple other creatures. In addition the 2nd time this triggers it should "double" the number of counters. Instead, even when a counter has already been added, triggering this ability the 2nd time only adds one counter instead of doubling them. So overall it seems the first +1/+1 trigger results in prompt for player to place the counter, but the system does not count it at all.
