"Athreos, God of Passage" Triggers incorrectly.
The text reads, "Whenever another creature you own dies, return it to your hand unless the target opponent pays 3 life."
I have noticed that trigger does not always happen, specifically when a creature is killed, and then exiled.
After more testing, I believe the issue is that the trigger is incorrectly set to occur when a creature enters the graveyard, instead of dies.

A creature entering the graveyard is the very definition of it dying. If it does not go to the graveyard, it does not die.
