Tarrian’s soulcleaver
Tarrian’s soulcleaver seems to be bugged. The trigger is supposed to be whenever a creature or artifact is sent to a graveyard from the battlefield, put a +1/+1 counter on equipped creature. However it activates constantly. Whenever I or my opponent cast any spell, as well as when a creature dies. It’s *****. Not exactly a common thing but annoying as * when I go against it.

The trigger condition is met, so the equipment triggers. Wether that trigger does anything useful or anything at all when it resolves is an entirely different matter.
