Stack is broken
I have been playing magic since Revised Edition and the rule has ALWAYS been that a countered spell has not been cast. And yet in Arena, effects that trigger off of a spell being cast are going on the stack even when the spell that triggered them has been countered. For example, if an opponent has Primal Bounty on the field and casts a creature, if I counter that creature then my opponent is still getting the free 3/3 Beast Token, even though no spell was cast. Triggered effects need to be removed from the stack when the source spell is countered.

Your understanding of the rules is seriously outdated. Countering a spell has not made that spell uncast since the great 6th edition rules overhaul in 1999, which also introduced the stack in the first place.
You can't counter a spell that was not cast or otherwise put on the stack. And countering a cast spell does not uncast it. Any triggers going off of casting a spell also go above the triggering spell on the stack, and resolve before it.
601. Casting spells

601.2i Once the steps described in 601.2a–h are completed, effects that modify the characteristics of
the spell as it’s cast are applied, then THE SPELL BECOMES CAST. Any abilities that trigger when a
spell is cast or put onto the stack trigger at this time. If the spell’s controller had priority before
casting it, they get priority.Nowhere in the procedure described in rule 601. for casting spells, does anyone have the opportunity to intervene with a counterspell. Counters can only be cast AFTER a spell was cast.
