Equipment falling of creatures
As an example, I was playing a game of brawl and had the "embercleave" attached to my "appa, steadfast guardian," and when I went to attach another equipment, which was "sword of forge and frontier," the embercleave automatically unattached itself from appa. I've had issues like this in the past with varying equipments. I know you are allowed to attach any amount of equipments to a creature but don't understand why arena does this.

Because that is the effect of protection from red, that your Sword granted to the creature: Red auras and equipment become illegally attached and fall off. Red auras go directly to the graveyard, and red equipment becomes unattached.
