Unstoppable Slasher bug.
Very often, not always, when destroyed by instant it doesn't return to the battlefield and it had no counters on it pre destruction.

First, did it actually die and was not exiled?
Second, cards like Grafdigger's Cage stop creature cards in graveyards from being brought to the battlefield.
Third, many removal spells do so by placing -1/-1 counters on the creature.
