multiple issues with Mirrorform in Lorwyn Eclipse quick draft
I have encountered a few bugs while playing Lorwyn quick drafts.
The first involved casting Mirrorform on Glamermite. Attached screenshot shows the board state after I spontaneously lost. I have no idea what caused it, but upon casting Mirrorform the defeat screen immediately popped up. My life total was positive, I had cards in deck, etc. There wasn't even a reaction from my opponent or anything, the defeat screen just appeared and I lost a match that was a certain victory.
The second - which may not be a bug and just confusing wording - also involved casting Mirrorform; this time on Trystan, Callous Cultivator. My permanents all transformed into Trystan, however the ability which triggers when he "Enters or transforms" did not trigger. Maybe that's just unclear wording, but Mirrorform turns permanents into the target, which I assumed counted as 'transform.'
Needless to say, I'll be avoiding drafting Mirrorform again, but I was sad to see an interesting deck lose to my own card on two occasions.

You tried to draw a card from an empty library, probably via your Voracious Tome-Skinner. That is a way to lose a game of Magic. It's how milling decks win.
And transform has a specifc meaning in Magic. It does not mean "becomes something else".
701.27a To transform a permanent, turn it over so that its other face is up. Only permanents
represented by double-faced tokens and double-faced cards can transform. (See rule 712,
“Double-Faced Cards.”)701.27e Some triggered abilities trigger when an object “transforms into” an object with a specified
characteristic. Such an ability triggers if the object either transforms or converts (see rule
701.28) and has the specified characteristic immediately after it does so.
