Bug caused creature to go to hand instead of graveyard
I killed a Saiba Syphoner with an Elsbeth's Nightmare but instead of going to the graveyard it went to their hand. I was unable to recover from that and lost the game.

I think the card was actually supposed to get shuffled into the library but instead went to their hand
