had kaya's ghostform attached to my commander, sacrificed it with witch's oven, and both commander and ghostform stayed in graveyard

Didnt activate when attached to my commander and commander died.

i use the combo (muldrotha + kaya's + altar of dementia + lotus petal) for a win con and this bug completely ruins it... please fix this

Same issue. Opponent used Elspeth Conquers Death to exile my Nissa with Kaya's Ghostform attached and Nissa didn't return from exile.

I also had multiple Kaya's fail against exile in a game

I had kayas ghost form attached to my grey merchat of asphodel and my opponent used elspeth defeats death on it to exile it and it did not cause kayas ghostform to trigger.

same issue with every spell that means: "..would die this turn, exile it instead"

As you can see here in User Voice, Kaya's Ghostform is simply broken and doesn't trigger in 1/2 of the cases it is designed to. Unless this card has gotten some kind of emergency ban, it's drastically weakening the card unintentionally, which has significant implications in gameplay when a large portion of the field is playing Jeskai Fires and UW Control, featuring multiple exile effects per deck.
Please fix the game engine with respect to this card and set us mono-black players free. We have a hard enough time in these match-ups without having to deal with an Arena-only bug leading to additional game losses!

I'm inching my way to Mythic level, but the dozen or so games this has cost me might well mean I fall short at the end.

This is a major bug. Have lost games directly from this. Usually on Eidolon of Hate for me. Killed multiple ways, it just does not trigger properly. Most recently when exiled via other enchantments.

Had Kaya's Ghostform in Gray Merchant and opponent cast Scorching Dragonfire(I had tried to weaken him to kill him, to trigger the return to BF, with Mires Grasp and opponent countered Dead Weight) Gary was exiled but didn't return as he should have.

This bug is a major break and it cost me the game. This bug has been prevalent and it has cost me many games and it's at the point where my deck is basically unplayable if I want to progress with it. This needs to be addressed ASAP.

Attached Kaya's Ghostform to Nicol Bolas, which was hit in a subsequent turn by Despark. I had the choice to send Bolas to the Command Zone or to Exile. I chose Exile and he was then exiled. Kaya's Ghostform didn't trigger. I was very tilted and did a rage scoop.
