Phyrexian unlife is not giving poison counters
I attack, deal damage, and the life total is negative but poison counters are never applied. This is not the first time I've seen this. Nothing else on the field is preventing the poison counters.

No Solemnity, no Vorinclex, Monstrous Raider? Are you sure? Especially Solemnity is the card played with Phyrexian Unlife.
