Caretakers Talent Card
I played an opponent and they were using Caretakers Talent from Bloomburrow. The first ability says that it can only go off once per turn and it went off the first time a token entered and the first time a token died, so it actually was going off twice a turn, and my opponent was getting two cards each turn from this card.

In general that once per turn is not working with caretakers talent you get automatic a card when a token enter the battlefield more than once a turn.
Known bug reported several times only retards play that card with the known bug -

Caretaker's Talent only goes off when a token enters the battlefield. If they also drew a card when a token died, then it wasn't from Caretaker's Talent.
