Incubator token bug
I had several Incubator tokens on the battlefield. I activated Progenitor Exarch to transform one of them. Instead of transforming, the token got sacrificed and I didn't get the creature.

I suspect your chosen token was a copy made with Ghired's ability. If so, the token had no +1/+1 counters to keep it alive once transformed. It died as a 0/0 creature.
