Agatha's Soul Cauldron Bug
Opponent had Tersa Lightshatter in play and had only Vivi exiled with Agatha's Soul Cauldron. Somehow, his Tersa used the activated ability of Voldaren Thrillseeker from his graveyard (sacrifice to deal damage equal to creature's power to any target) to kill me.
Expected behavior: creatures with +1/+1 counters should only have the activated abilities of the creatures exiled with Agatha's soul Cauldron.

Most likely Tersa exiled the Thrillseeker as the random card when it attacked, letting your opponent cast it after combat. The Thrillseeker's Backup trigger then granted Tersa the sacrifice ability and two counters.
