Opponent had 21 life. Vengeful Bloodwitchs ability triggered, bringing them to 20 life. Shouldn't Grievous Wound have brought them down to 10 life? This was their first time being dealt damage in the turn.

No, because loss of life does not equal damage. Damage to a player usually CAUSES loss of life, but not all loss of life comes from damage.
