All-Fates Scroll not correctly reducing cost based on unique land names
I was playing my final draft game when this bug caused me to lose the game by forcing me to tap 3 extra lands and not having the mana up to play depressurize, when I had manually tapped lands ahead of time.
Had 4 Forests, 2 Swamps, 2 Mountains, so All-Fates Scroll's tap ability should have costed 4 mana. I manually tapped four mana, leaving up 2 green, 1 black, and 1 red and then clicked the ability to sacrifice it for cards. However, it autopaid an additional three mana and left me with not enough mana up for my answers.

All-Fates Scroll's activation ALWAYS costs 7 mana, there is no cost reduction here. The number of differently named land you control only affects the number of cards you draw off the ability.
