Invisible removeall
My opponent used some card in the game that killed all my creatures. There was no animation, it didn't go to the opponent's graveyard, it wasn't exiled. I carefully examined every permanent on the board and there was no such effect; the card simply vanished. We played a little more, and he won quite predictably after a while, but I can't shake the feeling that he cheated somehow.

Scavenger Regent's Exude Toxin is an Omen. Omens are shuffled back into the library after resolving.
