Menace didn’t work.
In a standard game during my first main phase I played two copies of Grafted Butcher giving my three Phyrexian creatures menace (I had 2 Phyrexian artifact tokens in play and a Progenitor Exarch). My opponent only had two creatures that could block. When I attacked with my three creatures, my opponent first blocked two of my creatures with individual blockers then they switched their two blockers with the same two creatures they had blocked originally. This somehow allowed their two creatures to single block two of my creatures that had menace.
This is pretty serious bug which caused me to loose the game. I’m not sure if it’s something that happened specifically because of the scenario I described above, but it seems like something I think can be taken advantage of by other players that know how it works.

This sounds like they had the Brave the Sands enchantment out, allowing their two creatures to block two attackers each, thus allowing them to block two menace creatures with only two blockers.
