Shifting Wildwoods Bug
I activated “Shifting Wildwoods” targeting “Tamiyio, Collector of Tales” the ability resolved. “Shifting Wildwoods” went to the graveyard and did not become “Tamiyo, Collector of Tales”.

It DID become a Tamiyo, that's why it went to the graveyard. Because it became a planeswalker, but did not have nor get any loyalty counters, so it was a planeswalker with zero loylaty counters. And died as a state based action due to that. Turning Shifting Woodland into a planeswalker is a BAD idea.
Planeswalkers get loyalty counters by entering the battlefield or activating a loyalty ability, or get added counters due to some other effect like proliferate. But the Woodlands did not enter the battlefield, it was already there, just changing what it looked like. You could turn it into a surviving planeswalker, but only if you can get a loyalty counter on it BEFORE it becomes one.
