Boarded Window not working
I had two Boarded windows up and lost the match eventually because I could not block a creature attacking me because the boarded windows were not working on his creature and I would lose a much needed creature if I blocked his creature at full strength that should have been reduced by -2 power/

Blitz Not Working.
Blitz didn't work on creatures with blitz no other cards played that could prevent it
I had Cards with Blitz and the option didn't come up to cast them. I used the card " Riveteers Provocateur"

Grandmaster of the Great Fang Group, Fat Fang
My opponent tried to enter the battle phase when I had "Grandmaster of the Great Fang Group, Fat Fang" on my opponent's field and a unit in my opponent's Graveyard. I have a removal that can be cast (Mercurial Light March), and there should be a timing to hit this removal before entering the battle phase, but without that, I will arbitrarily say "Chief of the Great Fang Group, Fat Fang". induced.

Bug
Playing on my z flip 4 android phone. Play Mondrak Glory Dominus. Then, put Helm of the Host on it. Put Cathar's Crusade into play. Once you get 10 or 11 Mondrak's in play create a token. In this instance, I used Cathar's Call and had 11 Mondrak's in play. I created 240 token creatures. The creatures came into play just fine. The game froze, I'm assuming, while trying to calculate the +1/+1 tokens (240 of them) for each of the 251 creatures. The game clock goes into cycle then freezes with no time left and it defaults and ends…

Extraction Specialist did not return cost.

Learn Broken
The Learn mechanic is broken because you cannot discard a card from your hand as one of the options. This cost me a match because I needed to loot

Urabrask’s Annointer
During a Premier Draft match for Phyrexia All Will be One my opponent cast an Urabrask’s Annointer which is supposed to do damage equal to the number of permanents you control with oil counters on them to any target. My opponent only had 2 permanents with oil counters on them, but for some reason his Annointer did 3 damage to my creature, killing it and losing me the game.

Rending Flame extra damage bug
In game 2 of my draft event, I took 2 damage of Rending Flame when it targeted a flying human creature I controlled. A few turns later, I died to lethal damage which could have been blocked had I not taken 2 damage incorrectly.

Contagious Vorrac did not Trigger ability.
When I played Contagious Vorrac in a AllOne sealed game his when entering BF ability did not trigger.

Tyvar's Stand Not Casting.
Tyvar's stand is an instant of 1 green and x target creature gets hexpoof and indesructible. Upon ruthless predationing an opponents creature with reaction to stand. It wouldn't cast the stand costing me my creature and the match. Not sure why this is going on.

Veil of Assimilation does not activate
I put the escaped test subject in the field with the Veil of Assimilation on the field, but the effect did not appear.

Blade of Shared Souls
Unsure if this is a bug or rules issue admittedly, but the card Blade of Shared Souls when entering the battlefield, and targeting a 4/4 creature on my field to copy, unequipped itself instead of copying said 4/4. Tried to equip it to the 2/2 token it makes to try again, same issue. Again not sure if it's just a weird rules thing or a bug, but if it's the latter just wanted to let someone know. Lukka ability did not trigger.
I was playing a Phyrexia All Will Be One premier draft, and had Lukka, the planes walker. On my turn I needed to use his ability that dealt damage, which cost 4 loyalty counters and I had 5. However the ability didn't trigger and left me with no other chance to play the planes walker ability that turn, and I lost as a result.

I had 3 life

I blocked with a 6/5 and a 2/2
I took 3 damage and died

Multiple Master's Rebuke Not Castable
I was playing a match with 4 mana open and two masters rebuke in my hand with a fang of shigeki on the board, three green 1 white. I used my first master rebuke to kill a creature but then I never got a chance to use to the second one before ending my turn because the game skipped through my second main

Goldmaw Champion Mobile Bug
I was playing Kaldheim draft and when I attacked with an equipped goldmaw champion i was unable to click on it’s boast ability on mobile. The ability wasn’t even an option on the creature while equipped. I missed lethal and lost as a result.

Shocking Grasp did not work.
In Decathalon Omniscence draft, after my opponent cast Reckoner Shakedown, I was unable to cast Shocking Grasp (instant) even though there were multiple legal targets on the board. Because of this, I couldn't draw the Syphon Essence from the top of my library, and lost as a result.

Mind Rot in Omniscience Draft
Multiple times during this event I have played either counter or discard spells that simply do nothing. I have been playing magic for over 20 years, I am not saying I am incapable of error, but I scoured the battlefield and their was no spells preventing my mind rot. The opponent did not play any counter cards or anything. He did not discard any cards & it ended up costing me the match, which was my 3rd loss. This has happened multiple times during multiple omniscience drafts. I have tried to captured a log of each problem I have had…
